Fountain vs. Midi vs. Winona: As a Doctor, Which Menopause HRT Platform Would I Recommend?

As a doctor, I’ve seen how frustrating menopause care can be for women dealing with symptoms like sleep disruptions, brain fog, low energy, mood changes, and decreased libido.

Hormone Replacement Therapy (HRT) can be an option for addressing menopausal symptoms, but finding the right provider—and the right treatment approach—is incredibly important.

That’s why I took a closer look at three of the biggest names in virtual menopause care: Fountain, Midi, and Winona. Each offers convenient access to hormone therapy, but there are some meaningful differences in how they approach treatment and ongoing care.

Here’s how I’d compare them, and which one I’d recommend.

If You Want HRT That Truly Adapts to You… Choose Fountain

When I evaluate an HRT platform, personalization is one of the first things I look at. Hormone therapy shouldn’t feel like a standardized protocol because every woman’s symptoms, health history, and treatment goals are different.

Fountain starts with a comprehensive intake and face-to-face video visit before a provider creates a Personalized Hormone Therapy Kit around the patient’s individual needs.

More importantly, treatment doesn’t stop after that first prescription. Fountain incorporates ongoing video visits and lab monitoring so providers can adjust treatment as a patient’s needs change.

Midi and Winona also provide individualized care, but Fountain’s combination of customized medication, monitoring, and continued optimization stands out to me from a clinical-care perspective.

Winner: Fountain

If You Want a More Comprehensive Hormone Approach, Including Testosterone… Choose Fountain

Testosterone is an important part of the hormone conversation that can sometimes get overlooked.

Estrogen and progesterone tend to receive the most attention during menopause, but testosterone may also be considered for certain patients when medically appropriate.

Fountain allows providers to consider estrogen, progesterone, and testosterone as part of a personalized treatment approach based on the patient’s symptoms, health history, and clinical needs

For patients interested in discussing a more comprehensive hormone approach with their provider, I’d give Fountain the edge.

If Using Insurance Is Your Top Priority… Choose Midi

This is one area where Midi may have an advantage. Midi partners with many PPO insurance plans, which can make it appealing to patients who want to use their existing health benefits for menopause care.

However, insurance coverage doesn't necessarily mean every treatment option is covered. Coverage can vary based on the individual plan, medication, formulation, and other factors. This can be particularly relevant when considering  testosterone, which is often overlooked by insurance plans or not covered as consistently as other hormone therapies.

Fountain takes a different approach, offering an all-inclusive cash-pay membership covering provider visits, ongoing monitoring, medication delivery, and concierge support without relying on insurance coverage.

For patients whose top priority is using insurance, Midi may be worth considering. For patients who prioritize flexibility in their treatment options and a more comprehensive care experience, I'd recommend Fountain.

If You Want Consistent Monitoring and Adjustments… Choose Fountain

From a physician’s perspective, ongoing care is particularly important with hormone therapy.

A patient’s symptoms and response to treatment can change, which means the treatment that makes sense initially may need to be adjusted over time.

Fountain emphasizes follow-up appointments, video visits, lab work, and personalized medication adjustments rather than treating HRT as a one-and-done prescription.

Midi also offers ongoing care from menopause specialists, but Fountain’s combination of scheduled follow-ups, biomarker monitoring, and treatment adjustments makes its model particularly comprehensive.

If You Value Easy Access to Support… Choose Fountain

Patients inevitably have questions between appointments, so accessibility matters when I’m evaluating any telehealth platform.

Fountain provides 24/7 concierge support by text alongside ongoing face-to-face video visits with providers.

Midi offers provider messaging and specialist care, while Winona also offers physician messaging. But Fountain’s additional concierge support makes it especially convenient for patients who value having somewhere to turn when questions arise.

As a Doctor, My Overall Recommendation… Fountain

All three platforms can make menopause care more accessible, and I can see different patients preferring different options.

Midi stands out for women who want virtual menopause specialists while using eligible insurance coverage.

Winona provides a convenient, personalized approach to hormone therapy.

But if a patient asked me which of these three platforms offers the most compelling overall combination, Fountain would be my recommendation.

Its customized approach to hormone therapy, potential access to estrogen, progesterone, and testosterone when medically appropriate, ongoing lab monitoring, face-to-face provider visits, medication delivery, and 24/7 concierge support create a particularly comprehensive care experience.

Menopause treatment shouldn’t be about handing every woman the same prescription. It should start with understanding the individual patient and continue with monitoring how she responds.

That ongoing, personalized approach is ultimately why Fountain stands out to me.
